New Client Intake Form
Divine Nature Therapy ~ Please read and sign this Agreement.
The purpose of this letter is to clarify the nature of our relationship. You have chosen to meet with me in the context of Nature Therapy, Holistic Emotional Counseling, or Sacred Listening because you would like to explore your emotional, spiritual, and/or relational life, in whatever form that takes for you. I may use a variety of techniques to help you grow, heal, or discern as you express need to do. In my role, I adhere to the Guidelines of Ethical Conduct by Spiritual Directors International.
While you are absolutely welcome to discuss psychological difficulties in the context of our sessions, I am not a licensed mental health psychotherapist or psychiatrist, so I do not provide a clinical diagnosis nor prescribe medications, nor do I treat psychiatric disorders. I am happy to refer you to professionals providing those services as needed. You are welcome to utilize my services and psychiatric services concurrently. Similarly, you recognize that I do not provide financial advice and any financial decision and actions you make are done without my advice or recommendation and are purely your responsibility.
I will support you meeting your physical needs during our session; and, you take full responsibility for your comfort and safety, including possible hazards of meeting outdoors if we're outdoors, which include but are not limited to: insect or animal bites, sun or wind burn, and trips or falls.
You hold Shannon Gorres and Divine Nature Therapy harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with the performance of this Agreement.
Our conversations are confidential. Our time together is sacred. I will not reveal the content of our conversations or the names of my clients to anyone (with the possible exception of my supervisor Denise Ellen Hill) unless I am required to do so by law or court order, or it falls under mandatory reporting guidelines (child or elder abuse or suicidal intent). However, you are free to share whatever you choose about our sessions with anyone.
We commit to beginning and ending our sessions on time. If you wish to change an appointment, you can let me know at least 24 hours in advance by email or text. Without this notice, you are charged for the appointment time. If we are scheduled in person and either you or I have a mild illness (ex. a cold) or other issue, we could switch to an online session instead. If you have a bulk package, please follow the rescheduling guidelines. (Bulk guidelines: You can change one of your 6 appointments via email between sessions, and another may be rescheduled during your session, if needed. To accommodate possible illness or emergency, I allow you to reschedule on the same day, once a year, without charge.)
Therapeutic experiences and/or spiritual progress are dependent upon a variety of factors, including: what you bring to a session, the amount of time we have to work within a session, how you engage your life between sessions, and how sessions can build on previous work. Please bring your whole self! You are welcome to schedule once a week, twice a month, or once a month, or as needed, as you gain the resources from our sessions that uplift your life into greater wholeness.
I look forward to our time together!
~Shannon Gorres, MDiv, MA, ANFT certified guide
